The most common reason for this problem is the
"case" of the letters for the username and password have been
entered incorrectly. They are "case sensitive" (i.e., any
combination of upper or lower case letters). It may be necessary to
review the original documentation sent to you for the specific case configuration of
the username and password for your online account. If the problem
continues, contact John M. Baze Jr. in Computer Services (jmbjr@jmbbusinessforms2000.com).
Click the "PRODUCTS" link on the main navigation bar and find the category
where the specific academic form or its
product number is located. Click on the major group heading (e.g.,
"Grade Forms") and go to the form description or the associated
product number of the desired form. To view a larger representation of
that particular form, click on the smaller graphic above the form description.
A second method for locating a specific form, is to use
the website "Search" feature. Click on the Search button on
the top navigation bar. After the Search page has opened, type in the
form description or product number as instructed. A list of matched
possibilities will be displayed. From that list, click on the desired
form.

Click the "SERVICES" link on the main navigation bar and select "Business Forms Services." A
comprehensive list of other standard and custom business forms are represented
there.

Because of several industry factors (fluctuating paper
costs and competition from major competitors), the prices are not
listed. So, to get a quick price quote, a convenient Information
Request Form may be filled out from each major category web page (i.e.,
Services or Products).
All educational customers may set up a
"secure" online account from which prices of individual academic
forms are accessible. Computer forms for EDP or TECS academic programs
may be ordered on a regular or as needed basis. From this special area, a
customer may review past orders for a history of usage and to assist in
the determination of future budget needs.

You may need to update the browser by downloading the latest
version
(e.g., Internet Explorer or Netscape) that supports these Dynamic HTML Effects.

Any custom printing product for small businesses and large corporations can be
designed or reproduced. Click the
"SERVICES" link on the main navigation bar, then go to "Custom
Printing Services" to review some examples of many standard business
forms.
