Please Note: The best method for reproducing a master form is to send a CD, disk, or email the original form as an attachment in MS Word, Excel, Publisher (2000 or later versions), or PDF format. Also, include or identify all font types used on the original.  When mailing a master form, please print out the original form in the best printer setting and on a higher quality of paper and do not fold (if possible), staple, or use a paper clip.  If folding the original form is necessary, attempt to avoid folding on a graphic or major text area.  This will ensure that the highest quality reproduction of the form can be achieved during the printing process.  A faxed copy may be used for a price quote, but any low quality representation of an original form will have to be completely recreated which could result in additional charges added to the price quote.
